Heart&Stroke Lottery Rules & Regulations

By purchasing a ticket in the Heart and Stroke Foundation of Canada (the “Foundation” or “Licensee”) Heart&Stroke Lottery (the “Lottery”) and the 50/50 Draw of the Heart&Stroke Lottery, the purchaser acknowledges they have read, understood, and agreed to these rules and regulations (“Rules”). “Purchaser” means a person: (i) purchasing a ticket(s) for his/her personal use; (ii) purchasing a ticket(s) on behalf of a group where the purchaser’s name and up to two additional names and/or a group name appear on all ticket(s), or (iii) purchasing a ticket that is allocated to a group of which the purchaser is not a member. All names (maximum 3) within a ticket pack will appear on all tickets. The “Primary Ticket Holder” is the first name within the address section of the ticket. The “Primary Ticket Holder” and any additional names on the ticket are referred to as “Ticket Holders”. All Ticket Holders are subject to these Rules.

All inquiries regarding odds, conduct, prizing and procedures of the Lottery are to be made to the licensee prior to the purchase of ticket(s).

ELIGIBILITY

All Ticket Holders must be at least 18 years of age. Tickets must be purchased and delivered within the province of Ontario. Tickets cannot be purchased in the name of a minor. Employees and Directors of the Foundation, partners and employees of Deloitte LLP and its affiliates, employees of licensed gaming suppliers for this Lottery, and all persons domiciled with any of these individuals, are not eligible to purchase or hold a Lottery ticket. To be eligible, all ticket orders must be paid for and received, or post-marked by the respective Sales deadlines.

HEART&STROKE LOTTERY TICKETS

Tickets will be available until February 14, 2017 at midnight while supplies last. There are 131,171 tickets printed (125,000 available for sale and 6,171 to be awarded as prizes). Tickets are $100 each, 3-Pack for $250 or 5-pack for $375, and are not tax deductible. Ticket numbers will be issued in sequential order. Only one method of payment may be used for each order. Only tickets that are verified by the Licensee or its agents as paid for are eligible for their respective draws. Ticket(s) will be mailed (please allow a minimum of 3 weeks) to the address provided at the time of the order. It is the purchaser’s responsibility to ensure that ticket(s) are received before the applicable draws commence. The Foundation is not responsible for orders that are not received on time or at all. Tickets are only refundable prior to the draw date for which they are eligible. Ineligible requests will be denied. Notwithstanding anything contained herein to the contrary, the total liability of the Foundation to any Purchaser or Ticket Holder shall be limited to the purchase price of the ticket.

50/50 DRAW TICKETS  

All Rules applicable to the Lottery also apply to the 50/50 Draw. The winner of the 50/50 Draw prize will receive 50% of the total jackpot. There is a minimum guaranteed payout of CDN$500,000. The total jackpot is dependent upon the gross revenue generated from tickets sold. 50/50 Draw tickets are $10 each, 5-Pack for $25, or 15-Pack for $50. 50/50 Draw tickets must be ordered in conjunction with Heart&Stroke Lottery ticket(s). The total number of 50/50 tickets available for sale is unlimited based on the supply of Heart&Stroke Lottery tickets. 50/50 Draw ticket numbers will arrive on the same ticket as the Lottery ticket. In the event a Lottery ticket order becomes invalid, all 50/50 Draw tickets associated with the Lottery ticket will also become invalid.

NAME CHANGES

Any requests to change the name of any Ticket Holder must be received prior to the draw date for which the ticket is eligible and must be accompanied with the Purchaser’s Government issued photo ID. All other name changes will be denied with the exception of a Ticket Holder who becomes affiliated with the Foundation, Deloitte LLP or a gaming supplier for this Lottery, in a manner as set out in the Eligibility Section above, making them ineligible. If such situation arises, a name change request must be made to ensure compliance with the Rules.

SALES DEADLINES

VIP Prize Deadline is midnight November 10, 2016/Super Bonus Deadline is midnight December 14, 2016/Travel Canada Prize Deadline is midnight January 18, 2017/Early Bird Prize Deadline is midnight February 1, 2017/Final Sales deadlines is midnight February 14, 2017/50/50 Draw Sales Deadline is midnight February 14, 2017.

PRIZES All Values are in $CDN

There are 56,053 Prizes (“Prizes”) to be won, having a total value of $4,633,860.75 as of June 22, 2016 (including all taxes and freight). All Prizes are: tax-exempt for Ontario residents; may not be exactly as shown; may be subject to substitution of a prize of equivalent value and similar nature to be determined at the sole discretion of the Foundation; must be accepted as awarded; and must be claimed no later than ONE year after the respective draw date. Winners and all persons redeeming Prizes will be required to sign a waiver and release form releasing the Foundation and its affiliates and all others described in the said form of all liability in relation to any Prize awarded. Vehicle winners are responsible to license and insure the vehicle from the time of ownership. Unclaimed Prizes will be reported and a charitable allocation will be approved by the AGCO. Please allow a minimum of 4 weeks for delivery of Prizes, and note that vehicles may be subject to longer delivery times. No cash alternative is available unless otherwise specified.

There is ONE (1) prize for the 50/50 Draw. The winner will be awarded half of the 50/50 Jackpot.

Grand Prizes: There are 7 Grand Prizes. Grand Prizes 1 & 2: Cash prize of $1 million each; Grand Prize 3: 2017 Audi A4 Technik valued at $60,214 OR cash out = $50,000; Grand Prize 4: 2017 BMW 230i xDrive Coupe valued at $57,200 OR cash out = $50,000; Grand Prize 5: 2017 Ford F-150 XLT 4X4 valued at $46,000 OR cash out = $40,000; Grand Prize 6: Vacation for Two to Australia & New Zealand valued at $29,834 OR cash out = $25,000; Grand Prize 7: Cash of $20,000.

VIP Prize:: There are two (2) VIP prizes to be awarded, each valued at $12,500 OR cash out = $10,000. VIP Prize #1 is an Italy and Greek Island Cruise for two for 14 days. VIP Prize #2 is a trip for two to the U.K. for 4 nights to attend Wimbledon. Super Bonus Prize: Cash Prize of $20,000. Travel Canada Prize: Trip for two people across Canada valued at $50,000 or Cash Out = $45,000. All trip prizes include round trip departure from Toronto, accommodations, transfers and taxes. Early Bird Prize: There will be one winner of a 2017 BMW i8 valued at $189,118 plus $20,000 cash OR cash out = $200,000. There will also be 6,171 winners of one free Heart&Stroke Lottery ticket. Merchandise Prizes (50 prize draws including; 2 Samsung 65” 4K Ultra HD HDR Curved Smart TVs valued at $8,519 each, 1 Samsung 75” 1080p LED Tizen Smart TV valued at $3,773, 8 Dyson V8 Animal Cordless Stick Vacuums valued at $677 each, 15 Fitbit Alta Fitness Trackers valued at $192 each, 24 NOON VR Headsets valued at $146 each) must be accepted as awarded with no cash out option. Other Cash Prizes ((49,820 prize draws including 50 cash prizes of $1000; 1,000 cash prizes of $100; 3,900 cash prizes of $50; 44,870 cash prizes of $25) will be awarded as cheques payable to the Primary Ticket Holder. DRAWS

Winning tickets will be drawn under the supervision of Deloitte LLP. Prize draws for the Heart&Stroke Lottery will take place at 400 Applewood Crescent, Suite 500, Vaughan, ON. The 50/50 Draw will take place at the office of Deloitte LLP, Bay Adelaide Centre, East Tower, 22 Adelaide Street West, Suite 200, Toronto, ON. The records of Deloitte LLP shall be deemed correct and final. After each winning number is drawn and recorded, it will be returned to the drum for inclusion in all subsequent draws. The drum is spun at least 3 times between each draw with the exception of prizes valued at $1,000 or less where the drum will be spun after every 200th draw. The sequence of draws will be as follows: VIP Prize Draws (2): Nov 25, 2016 10am, beginning with the draw for VIP Prize #1, followed by draw for VIP Prize #2; Super Bonus Draw (1): Jan 4, 2017 10am; Travel Canada Prize Draw (1): Feb 2, 2017 10am; Early Bird Prize Draws (6172): commencing on Feb 22, 2017 10am, beginning with the draw for a 2017 BMW i8 plus $20,000 cash OR cash out, followed by 6,171 draws of one free Heart&Stroke Lottery ticket; draws for the remaining prizes will commence on Mar 6, 2017 10am, beginning with the cash prizes of lowest value to the highest followed by merchandise prizes of lowest retail value to the highest; Grand Prizes (7): Mar 22, 2017 10am. The 50/50 Draw will be held on Mar 23, 2017 10am.

NOTIFICATION OF WINNERS

All winners will be notified in writing. Major prize winners (greater than $10,000) may also be contacted by phone. A searchable winners list will be available at www.heartandstrokelottery.ca/winners on April 10, 2017. Government issued ID may be required for prize delivery. By claiming a prize, each prize winner consents to the use of their likeness (photograph, if available), first and last name, ticket number, city or town of residence and transcription/recording of the winner notification phone call to be used for any publicity purposes by the Foundation. Please allow a minimum of 4 weeks for delivery of Prize. It is the responsibility of the Primary Ticket Holder to allocate any winnings to other Ticket Holders.

The Foundation is only responsible for communicating with the Purchaser and is not responsible for any prize entitlement of any other Ticket Holder(s).

We are committed to protecting the privacy of your personal information. We will use your personal information to administer and evaluate the Lottery, including contacting, announcing and promoting Prize winners. In addition, we may use this information to send you donor-related communications and for tax receipting purposes, where required. Occasionally, we may contact you with mission-related or program-related communications. Heart&Stroke Cash Calendar Lottery Rules & Regulations

By purchasing a ticket in the Heart and Stroke Foundation of Canada (the “Foundation” or “Licensee”) Heart&Stroke Cash Calendar Lottery (the “Lottery”), the purchaser acknowledges that they have read, understood, and agreed to these rules and regulations (“Rules”). “Purchaser” means: a person; (i) purchasing a ticket(s) for his or her personal use; (ii) purchasing a ticket(s) on behalf of a group of people where the purchaser’s name with up to two additional names and/or a group name appear on all ticket(s), or (iii) purchasing a ticket that is allocated to a group of which the purchaser is not a member. All names provided and/or group name (maximum 3 names) will appear on all tickets within a ticket pack. The first name appearing in the section of the ticket entitled ‘Ticket Holder(s)’ will automatically be designated the “Primary Ticket Holder”. Each name following the Primary Ticket Holder name will be designated a “Secondary Ticket Holder” (Primary and Secondary Ticket Holders are referred to jointly as “Ticket Holder(s)”). All tickets held by all Ticket Holders are subject to these Rules.

GIFTING It is the sole responsibility of the Purchaser to track distribution of ticket number(s) and allocate any winnings. Only the Purchaser will be notified of winnings. Cheques will be made payable to the Primary Ticket Holder and mailed to the address provided by the Purchaser at the time of ticket order.

Prizes are tax-exempt for Ontario residents; may not be exactly as shown; must be accepted as awarded; and must be claimed no later than ONE year after the respective draw date. There is one (1) Holiday Draw Prize of $10,000 cash, twenty-seven (27) *Daily Prizes of $20,000 cash, four (4) Weekly Prizes in total; Week 1: $25,000 cash/Week 2: $50,000 cash/Week 3: $75,000 cash/Week 4: $100,000 cash, eleven (11) **Celebrate Spring Trip Prizes valued at $10,000 each or cash out = $8,500, two thousand five hundred (2,500) prizes of $30 Gift Cards from Indigo and one (1) Cupid Cash Prize of $15,000 cash. *Daily Prizes will be awarded every day in March with the exception of Mondays. **One (1) Celebrate Spring Trip Prize will be awarded each day beginning Mar 21, 2017 to Mar 31, 2017. Draw for the 1st Weekly Prize of $25,000 cash will occur on Mar 6, 2017; 2nd Weekly Prize of $50,000 cash will occur on Mar 13, 2017; 3rd Weekly Prize of $75,000 cash will occur on Mar 20, 2017; 4th Weekly Prize of $100,000 cash will occur on Mar 27, 2017. The 2,500 Gift Card Prizes will be drawn on Mar 1, 2017. Draws for the Celebrate Spring Trip Prizes will take place weekdays beginning Mar 21, 2017. Draw for the Cupid Cash Prize will take place on Mar 31, 2017. After each winning number is drawn and recorded, it will be returned to the drum for inclusion in all subsequent draws. NOTIFICATION OF WINNERS Beginning March 1, 2017, winners of cash and trip prizes will be posted every weekday (except holidays) on the Heart&Stroke Cash Calendar Lottery website at www.heartandstrokelottery.ca/winners and through social media. A searchable list of Gift Card Prize winners will be available at www.heartandstrokelottery.ca/winners on April 10, 2017. Winning notification to the Purchaser will be made in writing. Winning notification for major prizes may also be made by phone.You may also call 1-800-336-1334 or in the GTA 416-977-2201 to hear a recording of the winning names. Cheques will be made payable to the Primary Ticket Holder and mailed to the address provided by the Purchaser at the time of ticket order. Please allow a minimum of 4 weeks for delivery of cheques and prizes. Government issued identification may be required for prize cheque delivery.
